Fake Nurse Owned Med Spa, Allegedly Used Stolen RN License and Botched IV Treatments

The owner of a med spa in Watford City, North Dakota, has been arrested and charged after investigators say she falsely represented herself as a nurse while performing medical procedures, including IV infusions.
Kendra Cain, owner of Royals (Royal) Med Spa, does not hold a nursing or medical license of any kind. Law enforcement officials say Cain allegedly posed as a registered nurse and used another staff member’s professional credentials to operate the clinic.
About The Case
Police reports indicate that Cain is accused of:
- Using a licensed staff member’s credentials to schedule patients
- Order or prescribe medications
- Keep the clinic operating when the licensed nurse is not present
According to the police summary and local reporting, investigators say Kendra Cain “had been identifying herself to the public and to patients as a Registered Nurse” and representing herself as an RN with a license, even though checks of state and national databases showed she did not hold any nursing or medical license. They also allege she repeatedly added “RN” to her name in communications and used a staff member’s license to schedule patients, order medications, and perform procedures.
Patient Account Raised Safety Concerns
The investigation intensified after a patient, who is herself a registered nurse, reported concerns following an IV infusion at the facility.
According to police, the patient told officers that she had to coach Cain through placing the IV, and that the tourniquet was left on long enough for her hand to turn blue or purple. The patient also reported observing several inches of air traveling through the IV tubing toward her vein, raising concerns about the risk of an air embolism, a potentially life-threatening complication.
Search Warrant Findings
When officers executed a search warrant at the med spa on January 22, they reported finding a treatment area containing a used IV bag, IV tubing, syringe, needle, bandage, and an open sharps container. Investigators also noted what appeared to be fresh blood on the floor near the IV treatment area.
Police say checks of state and national licensing databases confirmed that Cain does not hold a nursing license or any other medical credential.
Charges and Court Date
Cain has been arrested and charged with multiple offenses, including:
- Practicing as a nurse without proper certification
- Reckless endangerment (extreme indifference)
- Unauthorized use of personal identifying information
- Deceptive writings
She was booked into the McKenzie County Correctional Facility. Court records show her next court appearance is scheduled for March 26. Her bond is set at $5,000.
The case remains under investigation.
