Nursing Program Director: Highland Valley Elder Services is looking for a Nursing Program Director to oversee all clinical assessment and eligibility programs. Directly supervise clinical staff. Responsible for establishing and maintaining working relationships with area home health agencies, health care facilities and skilled nursing facilities. Monitor and provide clinical support for the MassHealth Frail Elder Waiver Program, Enhanced Community Options Program and Community Choices Program. Directly supervise the ASAP RN staff and the CSSM Care Advisor. RN required with a minimum of 3 years experience in a community or long term care setting. Supervisory experience required. Valid driver’s license and reliable vehicle required. Excellent benefits including health and dental insurance, 401(k), generous time off package and competitive wages. Hours are Monday through Friday, 9:00 a.m. to 5:00 p.m. PRIMARY FUNCTIONS AND RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO: 1. Hire, Train, Supervise ASAP Nursing Staff and CSSM CA 2. Monitor the ASAP Personal Care Program. 3. Oversee the Clinical Assessment and Eligibility Program. 4. Provide clinical oversight of the Comprehensive Screening and Service Model (CSSM) and One Care Program. 5. Maintain statistics and performance reports for all home care and MassHealth programs. 6. Monitor and provide clinical support for the MassHealth Frail Elder Waiver (FEW) Program, Enhanced Community Options Program, and Community Choices Program. 7. Provide ongoing education and consultation to ASAP Case Management staff and area health providers regarding clinical eligibility criteria for ASAP and MassHealth programs and services. 8. Function as liaison to administrative staff at home health agencies, area skilled nursing facilities, and Executive Office of Elder Affairs clinical staff. 9. Directly supervise the ASAP RN staff and the CSSM Care Advisor. 10. Attend applicable staff and/or agency meetings. 11. Participate in training development activities to increase knowledge/skills for continuous strong performance in this position. 12. Other tasks as assigned by supervision to meet agency needs. 13. Participates in required quality performance initiatives within responsible program areas. 14. Assist Protective Service Program as back-up screener for intakes. 15. Perform other tasks as assigned by supervisor to meet agency needs. 16. Participate in training and development activities to keep up with knowledge/skills leading to strong performance in this position. 17. Participate in required quality performance initiatives. Work is 75% indoor office work; 25% outdoor (i.e. travel to attend required meetings, presentations and trainings). 3. Physical Requirements a. While performing duties of position, employee will be required to sit/stand for extended periods. b. Working knowledge of computers applicable software and databases and capacity to see computer screen and read written material. c. Capacity to communicate verbally with staff/supervisors, consumers and community. d. Capacity for the fine manipulation with use of office equipment. e. Capacity for occasional reaching, lifting or bending. 4. Work Environment Expectations a. Available to work a salaried position with primary hours daily M-F, 8AM-5PM. b.
