OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. SUBJECT TO FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS. VOID WHERE PROHIBITED.

1.  START DATE AND END DATE: The sweepstakes begins on 10/4/2023 at 12:00am Pacific Time and ends on 10/31/2023 at 11:59 pm Pacific Time

2.  ELIGIBILITY: Open to residents of the 50 United States and D.C., 21 years of age and older as of the start date of this sweepstakes. Officers, directors, managers, employees, and contractors of Full Beaker, Inc. (the “Sponsor”), and each of their parent companies, subsidiaries, affiliates and related companies and their respective advertising and promotion agencies, consultants and agents, as well as immediate family members and/or those living in the same household of each such person, and any others engaged in the launch of this sweepstakes (collectively, with Sponsor, the “Sweepstakes Parties”) are not eligible to participate. Social media platforms (e.g., Meta, TikTok, LinkedIn, YouTube, Twitter) do not endorse and are not sponsors of or affiliated with this sweepstakes. Void outside the 50 United States and D.C. and where prohibited.

3.  TWO WAYS TO ENTER: 

1. ONLINE ENTRY: Go to nurse.org/contest and follow the instructions to sign-up for the Sweepstakes. Fill in all required information. Your submission will represent one entry.

2. ALTERNATIVE FREE WRITE-IN ENTRY: On a postcard or a card that is no smaller than 3 ½ x 5 inches and no larger than 4 ¼ x 6 inches, legibly handwrite your complete name, mailing address, phone number and date of birth and mail with proper postage to: Full Beaker, Inc., Attention: Sweepstakes, 200 112th ave NE Bellevue, WA, 98004. All write-in entries must be postmarked by the End Date and received no later than 1 business day before the Drawing Date (see paragraph titled “Drawing” below). There is a limit of one (1) write-in entry per envelope/postcard. Each envelope/postcard is considered one (1) entry into the sweepstakes. Multiple entries may be submitted (not to exceed 21 entries). No mechanical reproductions will be accepted. Write-in entries must contain all entry fields outlined above; otherwise, they are void and will not be eligible to win.

Regardless of the entry method used, no more than 21 entries per person is permitted. 

Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different identities, and/or any other methods will void that entrant’s entry and that entrant may be disqualified from the sweepstakes. Entries submitted by anyone other than the entrant are void. In case of dispute as to the identity of any entrant, entry will be declared made by the registered name and address on such entry. Any potential prize winner may be requested to provide Sponsor with proof that such person is the prize winner. 

Use of any robotic, automatic, programmed, or similar entry method or entering more than the number of times permitted will void all entries and result in disqualification. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules. 

4.  DRAWING: A random drawing will be conducted on or about November 1, 2023 (the “Drawing Date”) to select the winners from among all eligible entries received. Odds of winning depend on the number of entries received. Potential winners will be notified by email and/or phone and must sign and return any required Affidavit of Eligibility and Release within 72 hours of the date notice is sent or such other date set by Sponsor. Failure to respond within the applicable time period will result in forfeiture of the prize and Sponsor will have the option to award the prize to an alternate winner. The return of any prize notification or prize as undeliverable may result in disqualification and an alternate winner may be selected. All income taxes resulting from acceptance of prize are the responsibility of winner. By entering this sweepstakes, entrant accepts and agrees to these Official Rules and the decisions of the Sponsor, which shall be final in all matters. All federal and state laws apply.

5.  LIMITATION OF LIABILITY: By entering this sweepstakes, entrants waive all right to, and release and hold the Sweepstakes Parties, Facebook and Meta, TikTok, LinkedIn, YouTube, Twitter harmless from, any claim, liability, loss, damage (including punitive, incidental, and consequential damages), or expense (including attorneys’ fees) arising out of or in connection with participation in this sweepstakes or the acceptance, use, or misuse of any prize. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. Sweepstakes Parties, Facebook and Meta, TikTok, LinkedIn, YouTube, Twitter will not be responsible for: late, incomplete, or incorrect entries; an entrant’s failure to receive prize notices due to entrant’s spam, junk e-mail, or other security settings or for entrants’ provision of incorrect or otherwise non-functioning contact information; technical, hardware, or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled, or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or used in this sweepstakes; by any human error which may occur in the processing of the entries in this sweepstakes; or any typographical, technological, or other error in the publishing of the offer, administration of the sweepstakes, or announcement of the prizes. If, in the Sponsor’s opinion, there is such an error, or there is any suspected evidence of tampering with any portion of the sweepstakes, or if technical difficulties (including viruses and bugs) compromise the integrity of the sweepstakes, Sponsor reserves the right to cancel or modify this sweepstakes in a manner deemed appropriate by Sponsor. In the event of termination, winners will be selected from among all eligible entries received as of date of termination. In the event a dispute arises as to the identity of a potentially winning entrant, entries will be declared made by the name on the online entry form.

6.  PRIZES: 

Winners will be drawn for the following prizes:

• SimpleNursing.com, 1 year membership (1 winner)

• MDF Instruments, ProCardial Titanium MPrint Stethoscopes (10 winners)
• Jaanuu women’s scrubs set (25 winners)
• Nurse Mates shoes and compression socks (3 winners)

Total approximate retail value of all prizes: US$4,990.  

The prize consists of only those items specifically listed as part of the prize.

Limit one prize per family or household. No substitution or transfer of prize permitted by winner. Sponsor reserves the right to substitute a prize of equal or greater value. All prizes will be awarded. If a potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner by random drawing from among all remaining eligible entries. Only three (3) alternate drawings will be held, after which the prize will remain un-awarded.

7.  CHOICE OF LAW AND FORUM: Entrant agrees that all matters arising out of or relating to this sweepstakes and these Official Rules are governed by, and construed in accordance with, the laws of Washington, without giving effect to any of its conflict of laws provisions thereof. Entrant further agrees that any legal suit, action, or proceeding arising out of or relating to this sweepstakes and these Official Rules shall be brought exclusively in the federal or state courts of the State of Washington, sitting in Seattle.

8.  PRIVACY: Information submitted with an entry is subject to the Privacy Policy located here: https://nurse.org/pages/privacy-policy/. By entering via our online method, you consent to receive our weekly newsletter via email. 

9.  WINNERS LIST: For the names of all prize winners, available after 11/8/2023 that is 1 week after end date, send a self-addressed, stamped envelope with your request to: Full Beaker, Inc., Attention: Sweepstakes, 200 112th Ave NE Bellevue, WA, 98004.
