Nurse PRN needs a RN Case Manager RN in Falls Church, VA - The RN Case Manager is responsible for planning, coordinating and facilitating medical care for patients. It is crucial that Registered Nurse Case Managers focus on providing quality medical services and efficient processes for the organization and patient.
Responsibilities:

A Registered Nurse Case Manager's responsibilities include:Communicating with the patient and their family to identify their needs.Developing approach to satisfy the patient’s needs.Providing resources to the patient and their family.Help in finding the most affordable method of care.Reaching out to make sure the plan is exceeding the needs of the patient.
Requirements:

One year of experience as a Registered Nurse Case Manager within the past two years. Active RN License in good standing.Current Basic Life Support through American Heart Association.Negative Drug Screen.
