LPN - Intake & Office Coordinator

American Care Partners (ACP) is a Premium Home Care Services and Assistance Provider in Northern Virginia. We are a Joint Commission  accredited, Medicare & Medicaid certified home health agency. We provide  Skilled and Non-Skilled (Non-Medical) Home Care Services & Private Duty Nursing for pediatric and adult patients. We are seeking a Licensed Practical Nurse (LPN) to work internally in the office and out in the field to support our various clinical programs.  This position wears many hats as we are a dynamic agency that is growing rapidly.  Applicants of this position will need to be flexible and have the ability to multi-task and work in a high paced, high-stress environment.

Job Duties:

  • Handle clinical duties of intake process

  • Submit authorization documents to payer sources and interpret orders for authorization justification

  • Coordinates with physicians and medical directors regarding ACP patients

  • Conduct routine follow up visits on pediatric patients

  • Perform clinical interviews with ACP applicants

  • Assist Directors of Nursing with administrative and educational duties

  • Acts as an internal clinical liaison between referral sources and ACP

  • Files medical records as needed

  • Other duties as assigned.

Minimum Qualifications:

  • At least  3 years of nursing experience

  • Prior experience with pediatric patients in a home, hospital or rehab setting

  • Current CPR Card Certification

  • Active, unencumbered licensure as an LPN in the State of Virginia

  • TB test results or chest X-Ray

  • Prior experience with basic computer applications

Preferred Qualifications:

  • Prior experience in a customer service setting

  • Health Plan Experience

  • A ‘Winning Attitude’

  • Previous discharge planning experience

  • Dynamic and magnetic personality

  • Experience as admissions coordinator/director at Rehab or SNF
